17-01-01-02. Fees. The board charges the following fees: 1. For an application for initial licensure, three hundred dollars. 2. For renewal of a license, four hundred dollars for active status or one hundred dollars for inactive status. 3. To change from inactive to active status, three hundred dollars. 4. For a duplicate license, twenty-five dollars. 5. The additional administrative fee for late renewals is three hundred dollars.
