43-02-02-12.3. Construction quality assurance plan. 1. The operator shall develop, for the department's approval, a construction quality assurance plan that addresses all aspects of constructing surface facilities. The plan must include the following: a. A description of the responsibilities and authorities of key personnel, including the personnel's level of experience and training; b. A description of the required level of experience, training, and duties of the contractor, the contractor's employees, and the quality assurance inspectors; c. A description of the testing protocols for every major phase of construction, including the frequency of inspections, field testing, and sampling for laboratory testing; d. The sampling and field testing procedures and the equipment to be used; e. The calibration of field testing equipment; f. The laboratory procedures to be used; and g. A description of the documentation to be maintained. 2. The operator shall submit the construction quality assurance plan at the same time the plans and specifications required in section 43-02-02-12.2 are submitted. History: Effective July 1, 2013.
