66-02-01-08. Fees. A completed application initiation form and paid fee must be on file with the board prior to beginning practice under provisional licensure. Failure to pay the annual licensure renewal fee by November 15 of each calendar year will delay renewal issuance and require the cessation of practice during any period of time the individual has not been issued a valid license.
