72-05-01-04. Mandatory transmission cover sheet. A facsimile filing must be accompanied by a completed transmission cover sheet. The cover sheet must be the first page transmitted. The state agency shall ensure that any credit card information on the transmission cover sheet not be publicly disclosed. History: Effective October 1, 1998.
