42-02-02-02. Procedures for application. 1. Applicants shall submit: a. A completed application form; b. Verification of tribal enrollment; c. The applicant's most recent transcript that shows a cumulative GPA, or its equivalent; d. A needs analysis form completed by a financial aid officer at the institution the applicant will be attending; and e. The free application for federal student aid (FAFSA), for consideration of need-based awards. 1 2. Current award recipients may apply for the scholarship in subsequent years by submitting a new application and the items listed in subdivisions c through e of subsection 1.
