89-07-02-21. Restoration of permit. At any time after the suspension, revocation, or modification of a permit, the department may restore it to the permittee or delete any modification of the permit upon a finding that the requirements for issuance of an original permit have been met by the permittee or that the conditions requiring modification no longer exist.
